How to Apply for Boards & Committees

To be considered for a board or committee, you must complete an application form and submit it to the City Clerk’s Office.

Apply OnlineComplete and submit your application online.

While submitting your application online and after you click "Submit" or "Submit and Print",  you  should see a "Form Submission Confirmation" page thanking you for submitting your application.  If you do not see this confirmation page,  please call the City Clerk’s Office at (904) 247-5809 to confirm your application has been received.
Apply in Person or by MailDownload the Board/Committee Application Form, complete the form, then  do one of the following:

  • Bring it to the City Clerk’s Office, 800 Seminole Road, Atlantic Beach, FL 32233, Monday through Friday, between the hours of 8 am to 5 pm;
  • Mail it to the City Clerk’s Office, 800 Seminole Road, Atlantic Beach, FL 32233;  or
  • Fax it to the City Clerk at (904) 247-5846.
