Atlantic Beach Dog Park

Please print and complete the Application, Waiver and Park Rules form and the Record of Shots form below. Shot records must be completed and signed by a Veterinarian before access cards and dog tags will be issued. Bring completed forms to the Cashier's Counter at City Hall at 800 Seminole Road with your payment.

Annual usage fee $50 for the first dog, $25 for each additional dog. Lost card replacement fee is $15. (Sales tax will be added to all fees).
